Yes, we have clinicians who accept the insurance plans listed below.
Please note that we are not in-network with any Ohio Medicaid plans.
Accepted insurance includes Aetna, Cigna, Devoted Health, GEHA, Humana, Medical Mutual, Ohio Healthy, Optum, Oscar, OSU Student Insurance, United Healthcare, UMR, Zelis, and other subsidiaries of the plans listed above.
Anthem is accepted by our Speech-Language Pathologist, Occupational Therapist, Jason Waller, and Spinisha Jetson, but not by our psychologists.
We also welcome self-pay clients and can provide superbills for you to submit to your insurance company to request reimbursement if we are considered out-of-network.

Counseling: $150-$265 per hour, depending on the clinician
Comprehensive Psychological Evaluations: $1800-$3000 total, depending on various factors

We typically accommodate intake and feedback meetings for evaluations virtually. However, at least one in-person appointment is required for the testing portion. For clients traveling long distances, we can often consolidate testing into a single 4-6 hour session instead of the usual 2-3 two-hour sessions. Note that some insurance companies will not cover virtual feedback meetings. Check with your insurance provider for more information about your personal benefits. Step 1: Initial Intake Appointment
Duration: Approximately 1 hour
Description: The psychologist will conduct an interview with the client or the parent(s) of the client, and decide which assessments to perform based on the information provided.
Step 2: Assessment and Evaluation Appointments
Duration: Approximately 4-6 hours total, split into two to three 2-hour sessions.
Description: The psychologist will perform multiple face-to-face assessments with the client at these appointments.
Step 3: Follow-Up Appointment to Review Results
Duration: Approximately 1 hour
Description: Two weeks after testing is completed, the psychologist will provide a detailed written report including the results of all assessments, relevant diagnoses, and recommendations.
Cost and Payment Information
The cost of the evaluation varies based on your payment method and insurance coverage. If you choose to self-pay, the rate ranges significantly depending upon the referral concern and depth of assessment needed. However, if we are in-network with your insurance, your out-of-pocket expenses will likely be significantly lower. We recommend contacting your insurance company to determine their contracted rate with us, and we can also provide a rough estimate.
AutoPay must be set up with a credit card as per our treatment contract to secure your appointment times. Client balances will be charged to the credit card on file once the testing is completed and your insurance company has responded to the claim.

Do you provide ESA letters? In very limited situations. ESA letters are only considered for current, ongoing, long-term therapy clients when we have a well-established therapeutic relationship.
Why is qualifying for an ESA letter “very rare”? ESA recommendations must be clinically sound and in the best interest of both the client and the animal. That bar is high by design. We only recommend ESAs when there is clear, sustained clinical rationale developed over time in treatment.
What counts as “ongoing, long-term” care? Care that has occurred across multiple sessions over time, where your clinician knows your history, symptoms, progress, and treatment plan well enough to make a responsible recommendation. Brief, drop-in, or single-session care does not qualify.
Can I get an ESA letter if I’m a new or short-term client, or if I only need the letter? No. Short-term clients and one-time requests do not meet our threshold for an ESA recommendation.
If I don’t qualify for an ESA letter, can you suggest other supports? Yes. Speak with your therapist about evidence-based supports and accommodations that may meet your needs without an ESA letter. We won’t leave you guessing.

Will you complete FMLA paperwork? Yes, within a clear scope: we will complete FMLA forms to cover the dates and times of your (or your child’s) scheduled appointments with Achieve.
Can you certify that I have a disability or excuse me from work entirely? No. Certifying a disability that excuses you from work requires a formal disability evaluation completed by a physician. That determination is outside our role.
Can you support intermittent leave beyond appointment times? We can document and certify only the actual dates and times of your Achieve appointments. Requests for additional time away from work beyond those appointments must be addressed with your physician or employer under their policies.
What can you provide instead of disability certification? We can provide requests for reasonable accommodations at work or school related to your care at Achieve (for example, scheduling flexibility to attend therapy). These requests are tailored to treatment needs and are not disability determinations.
My employer is asking for more detail. What can you share? We can confirm appointment attendance and provide a brief treatment-related accommodation request when appropriate. We do not share sensitive clinical details without your written authorization, and we do not certify disabilities.
